Evergreen will offer FREE well child checks and sports physicals for all school-aged kids. They will also offer all school required immunizations during the visit. Insurance will be billed, but there will be no out of pocket costs to families. Don't have insurance? No worries! They will help get you in touch with one of their CHW's to assist in signing up for the Oregon Health Plan.
